Please take your time when filling out the information required below and please don’t forget to READ and SIGN the liability release at the bottom of this contract

LIABILITY RELEASE

I am aware of the inherent hazards of scuba diving and understand that I should:

• Be in good mental and physical condition for diving.
• Avoid being under the influence of alcohol or dangerous drugs while diving.
• Engage only in diving activities consistent with my training and experience.
• Listen carefully to the diving briefings and direction given by the dive master and follow these closely.
• Adhere to the buddy system throughout the dive.
• Refrain from exceeding a maximum depth of 80 ft. Unless otherwise previously scheduled.
• Make a preliminary beach dive if I have not been diving for the past six months.
• Respect the reefs of Cozumel and the laws that protect them by not having any physical contact with them diving.
• Make a check out dive one day before doing a boat dive if my last dive was more than 6 months ago.

NOTE: A private dive master will be required ( additional fee) for Junior open water divers with age of 10 to 11 years old on all 2 tank boat trips to a maximum depth of 40 feet.
For Junior open water divers with age of 12 to 14 years old a private dive master is required and the maximum depth for them is 60 feet.
